The voices of our community are so important when it comes to planning for the future. You can help us to identify emerging health needs and models of care so we can plan to provide this care in a timely manner, closer to home.

The survey and consultation sessions will help document the evolving service requirements for the Grampians Health Horsham area and be compared against the models of care outlined in the Grampians Region Health Services Plan. This comparison will help refine plans for future services and ensure they align with the objectives of Grampians Health and the needs of the community.
